the Verification Room

A verification annex, not a separate canonical stack stage. It exists to support Execution, Improve, and the Archive when an outcome needs an explicit evidence check. Compare the observed result against the done condition established in Discipline and the execution evidence. DISPOSITIONS DONE — acceptance criteria supported by inspectable evidence. DONE_WITH_CONCERNS — criteria supported, with named limitations or maintenance concerns. NEEDS_CONTEXT — evidence insufficient to establish the result. BLOCKED — a named prerequisite or authority boundary prevents verification. FAILED — intended effect did not occur or acceptance criteria were not met. Failure is a result. Concern is a result. Unknown is a result. Do not reward effort by changing the standard. Where feasible, verification should be independent of execution; where that is impractical, record the limitation honestly. Verified outcomes route to Improve for correction extraction or justified preservation, then onward to Maintenance.
